According to the report, substance use disorders result from changes in the brain that occur with the repeated use of alcohol or drugs. These changes take place in brain circuits involved in pleasure, learning, stress, decision-making and self-control. Health care professionals use criteria from the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), to assess whether a person has AUD and to determine the severity, if the disorder is present. Severity is based on the number of criteria a person meets based on their symptoms—mild (2–3 criteria), moderate (4–5 criteria), or severe (6 or more criteria).
People with severe or moderate alcohol use disorder who suddenly stop drinking could develop delirium tremens (DT). It can be life-threatening, causing serious medical issues like seizures and hallucinations that require immediate medical care. Many people with AUD do recover, but setbacks are common among people in treatment. Behavioral therapies can help people develop skills to avoid and overcome triggers, such as stress, that might lead to drinking. Medications also can help deter drinking during times when individuals may be at greater risk of a return to drinking (e.g., divorce, death of a family member).
